> Letter of recommendation must include the following:
• Applicant’s name
• Recommender's relationship to the student

> Written essays must be 300-500 words, and respond to one of the following prompts:
• Prompt 1: Identify and explain what adjustments will be necessary for you to achieve success in college, and what steps you have taken to make those adjustments.
• Prompt 2: Identify one problem that exists, either locally or nationally, and explain how you could use your degree of choice to eradicate the problem.
• Prompt 3: Oscar Washington, Jr. valued education and was committed to helping students achieve academic excellence. Explain why you value education, and what it means to you.

All information below, including information regarding gender, race, and ethnic origin, is optional. The information is requested for post-scholarship report generation only and will NOT be considered when evaluating applicants, nor will in any way affect your selection for the scholarship.

Please list the costs of tuition, lodging, and board, and books and supplies for the top two colleges you are considering. If you are not considering a second college, please enter the value "0" into the College #2 fields.
